Dental Form app for iPhone and iPad

Dental Form is a simple and easy to use patient registration app which is customized for dental offices. There is no need to change the workflow or computerized your dental office. This app is compatible by the legacy paper-and-pen system which means both approaches generate similar documents. This is great as your patients will be given options: a faster, easy to correct, and more fun experiance (finger and app) or the traditional approach (pen and paper). All you need is one iPad and one Airplay printer.
Each patient will be guided to complete the form which asks for the following information: Personal Information, Payment Information, Medical History, and Dental History.
At the last step, the patient can use their fingers or a stylish pen to sign the form. The app generates a two-pages PDF which can be printed by an Airplay printer.

The patient information are processed locally and neither stored in the app nor sent to any servers.
